This program is primarily designed for future EMS personnel, but is appropriate for anyone who may respond to emergencies including fire/rescue personnel, nurses, coaches or athletic trainers, safety professionals, etc. Participants who successfully complete this program are eligible to take National Registry of EMTs certification exams for PA and national certification. FIRST SESSION: Tues. 8/18 6-9PM COURSE DATES: 8/24 – 12/6/2015 LAB SESSIONS: Sat/Sun 9-5 on: 9/26-27; 10/24-25; 12/5-6 (All sessions mandatory) LOCATION: EMMCO-West Office: 16271 Conneaut Lake Rd. Meadville, PA 16335 COST: $600 (textbook and online access code included) *additional fees as required for NREMT exam fee and clinical contacts for unaffiliated students (see page 2) This hybrid EMT course is designed for students who are self-motivated, academically disciplined and technologically proficient with standard computer applications. Students will complete required course lessons online each week and attend skill labs on scheduled days. See reverse for technical requirements, list of co-requisites and additional fees not included in the course cost. Students must be at least 18 years of age to enroll. PREREGISTRATION is required. To enroll please complete the form below and return it with a $200.00 nonrefundable deposit (check or MO payable to “IUP”) by 8/7/15. The remaining fee ($400 check/MO payable to IUP) will be due at the first class session. Students without good academic or computer skills should consider a traditional classroom format instead. Co-requisites: *In addition to online and lab sessions students must complete 10 patient contacts at an approved field site. This will require: Up to date immunizations and current negative TB test (cost varies) PSP criminal clearance ($10) Malpractice insurance at student rates ($38 www.hpso.com) *Costs or fees to meet these requirements are not included in the course fee. Students must also complete these co-requisite courses to be eligible for PA EMT certification exams: • IS 100.b (available free online) • IS 700.a (available free online) • Hazardous Materials Awareness course Criminal History: Students must complete a criminal history form for PA DOH upon enrollment in the course. Students with a positive criminal history may not be eligible for EMT certification or field/clinical experiences.
